Use this form to request reimbursement for funds
awarded for 2012. Request for Payments are accepted monthly.

2012 Event Follow-up Report (PDF 32KB)
This form must accompany your final 2012 Request for Payment if grant was awarded for a specific event.

2012 Sport / Event Funding and Guidelines (PDF 37KB)
Use this form to request funding assistance for a specific sporting or community event. Sport / Event funding applications are accepted monthly on a first come first served basis.
